Job description
Gentiva Hospice is seeking a Patient Care Manager to oversee clinical operations, coordinate care across the interdisciplinary team, and mentor staff to uphold quality and regulatory standards in home-based hospice care.
Reporting to the Executive Director, you will manage visits, triage referrals, lead IDG meetings, ensure compliance, and drive performance improvement while supporting compassionate care for patients and families.
