Job description
Heartland Hospice, part of Gentiva, seeks a Patient Care Manager to lead clinical operations and ensure compliant, high-quality hospice services in our local team.
Reporting to the Executive Director, you will oversee patient care, manage visits, triage referrals, lead IDG meetings, mentor staff, and drive regulatory compliance. This role requires RN licensure in WI, 3+ years direct care including hospice, and strong leadership to support compassionate care.
