Job description
Heartland Hospice is seeking a compassionate Patient Care Manager, RN to lead and coordinate interdisciplinary hospice services for patients with life-limiting illnesses. In this role, you will oversee clinical care planning, conduct patient assessments, manage case loads, and ensure high-quality pain and symptom management in home and facility settings. You will supervise and mentor nursing staff, collaborate closely with physicians, social workers, chaplains, and therapists, and support families through education and emotional guidance. The ideal candidate is an experienced hospice or home health RN with strong leadership, communication, and organizational skills.
