Job description
Gentiva Hospice seeks an experienced Patient Care Manager to lead clinical operations, coordinate care, and mentor staff across hospice services. You will ensure compliant, high-quality patient care, oversee scheduling, triage referrals, and guide interdisciplinary teams.
A strong emphasis on regulatory adherence and performance improvement is required. The role requires an RN license, a nursing degree, and at least three years of direct patient care experience, including hospice or home health.
