Job description
SouthernCare is seeking a Patient Care Manager to oversee clinical operations, coordinate care, and lead staff to deliver compassionate, compliant hospice services. You will report to the Executive Director and drive quality, regulatory adherence, and performance improvement across the team.
The role requires an RN license in Indiana, 3+ years of direct patient care (2+ in hospice/home health), and proven leadership in a fast-paced environment. BSN preferred and EMR proficiency are beneficial.
