Job description
Optum is hiring a Physical Therapist to provide hospice services in Oregon. The role includes assessment, direct treatment, home exercise program development, patient/family instruction, and documentation per hospice standards.
Responsibilities include evaluation initiation, care plan collaboration with the Interdisciplinary Group, and discharge summaries. Requires Oregon PT licensure, CPR, and reliable transportation. A $10K sign-on bonus is available for eligible full-time candidates.
