Job description
Gentiva is seeking a Hospice Physician Designee to provide compassionate medical leadership for patients receiving home-based hospice care. In this role, you will oversee symptom management, conduct patient assessments, and guide individualized care plans that promote comfort, dignity, and quality of life. You will collaborate closely with an interdisciplinary team, support families through end-of-life decisions, ensure regulatory and clinical compliance, and contribute to a culture of empathy, teamwork, and clinical excellence across our hospice programs.
