Job description
LifeBridge Health, Inc. seeks a Hospice Physician to provide compassionate, patient-centered care to adults with serious, life-limiting illness across central Maryland. In this role, you will manage complex symptoms, lead goals-of-care and advance care planning conversations, and collaborate with an interdisciplinary hospice team to develop individualized care plans. Responsibilities include rounding in home, inpatient, and facility settings; coordinating with primary and specialty providers; ensuring regulatory compliance; documenting in the EMR; and participating in quality and performance improvement initiatives.
