Job description
Hartford Healthcare seeks a Hospice Quality Assurance Performance Improvement Coordinator to lead quality initiatives for our home hospice program. In this RN role, you will monitor clinical outcomes, perform chart audits, analyze trends, and develop performance improvement plans that ensure regulatory compliance and excellence in end-of-life care. You will educate interdisciplinary hospice staff on QA/PI standards, support survey readiness, and partner with leadership to implement evidence-based best practices.
