Job description
Heartland Hospice, part of ProMedica, seeks an Afterhours RN Care Manager to provide triage, crisis intervention, and skilled nursing care to hospice patients during evenings, nights, weekends, and holidays. The RN coordinates symptom management, pain control, and emergency visits in home and facility settings, ensuring comfort, dignity, and support for patients and families. Responsibilities include phone triage, assessment, implementing care plans, collaboration with the interdisciplinary team, accurate documentation, and providing emotional education and support aligned with hospice philosophy and regulatory standards.
