Job description
Hartford Healthcare is seeking a Float Hospice RN for weekend coverage to provide compassionate end-of-life care in patient homes across our Connecticut network. As a weekend case manager, you will conduct thorough assessments, manage complex symptoms, and coordinate individualized care plans with an interdisciplinary team.
