Overview
Community Health Network (CHN) has been placing experienced nurses and physical therapists in home care, hospice, and case management for over 30 years. We offer flexible work options and outside of the box opportunities that focus on your needs and interests.
Role
We are seeking an experienced Hospice RN Case Manager to provide care to patients in a mix of long‑term care facilities and homes.
Location
Coverage will be in Chelsea, Winthrop, Everett, MA and surrounding areas.
Qualifications
Requires a minimum of 1 year of Hospice RN experience with a Medicare‑certified agency.
Responsibilities
Provide quality nursing care to identified Hospice patients.
Perform direct patient care, coordinate services, and advocate for patients.
Engage with patients, families or significant others, and the community.
Comfortable handling end‑of‑life issues, death, and dying.
Schedule
Full‑time, Monday‑Friday. No weekends, holidays or on‑call required.
