Sage Group HealthCare Staffing is seeking a Hospice RN CM or a 13-week travel/contract position in Tullahoma, TN. Accepting both local and traveler candidates.
Position Details:

Contract Length: 1 3 weeks
Pay Rate: Range $2000 to $2080 Weekly Gross pay based on a 40-hr work week (Includes Non-taxable travel/housing and M&I stipend available for qualified/eligible travelers)
Mileage Reimbursement: $0.49/mile for distance covered between patient home appointments
Unit: Home Health, Hospice
Shifts available: Days
Shift timings: 8 Hours; Days, 8:00 am - 5:00 pm
Schedule: 40 hours/week guaranteed, 5 shifts per week, Mon-Fri, 1 night/week per call coverage
Position Requirements:
Registered Nurse: 2 years - Required
Recent/Current Hospice nursing in a Home Health setting: 1 year within the last 3 years - Required
Home Care Home Base (HCHB)charting, hands-on experience and proficiency: 1 year - Required
Must have valid Driver's Licenseand Personal Auto Insurance
Must be Covid Vaccinated orprovide medical or religious exemption
Education, License and Certification Requirements: ASN or an equivalent - Required
BSN or an equivalent - Highlypreferred
RN License for State of Indiana or Compact -Required
BLS Certification from AHA orAmerican Red Cross - Required
Job Description: Provide and coordinate care forassigned hospice patients in comfort of their homes.
Develop and implement nursingcare plans including Start of Care and admissions as assigned.
Provide case management services for the assigned patient case load.
Support pain management andability to stay at home - enabling them to spend quality time with theirloved ones and doing the activities they enjoy
Empower patients and families toreach healthcare goals by educating them on disease management
Case management and provideclinical oversight to interdisciplinary teams
Provide nursing care and serviceswithin the standards and scope of nursing practice as defined within thestate Nurse Practice Act regulations, policies, procedures, and protocols,as well as facility and corporate policies and procedures.
Ensure physicians' orders arelegible if taken verbally and transcribed appropriately per policy andprocedure.
Ensure patients' status isassessed on an ongoing basis and pertinent information is gathered anddocumented.
Notify physicians of significantchanges in status
Document and complete chartingfor all patient visits within 24 hours.
