Job description
Optum, through Star Hospice and LHC Group, is hiring a Registered Nurse Case Manager to oversee hospice patient care. You will develop and implement the Plan of Care, evaluate performance, and coordinate care with the care team.
This full-time role (also PRN) requires RN licensure, CPR, and strong communication. A sign-on bonus of $5,000 is offered, along with a comprehensive benefits package and retirement options.
