Job description
Chapters Health System in Florida is seeking a registered nurse to manage clinical admissions for hospice services. You’ll explain care options to patients and families, answer questions, and begin the Plan of Care in collaboration with physicians and the IDG.
You’ll initiate skilled nursing interventions to prevent complications, obtain physician orders, and ensure accurate documentation and safe discharge planning in alignment with company policy.
