Job description
Alc-Health-Care-Services seeks an experienced hospice RN to plan, organize and direct home-based hospice care, drawing on community resources to meet patient and family needs in their homes and communities.
The role involves comprehensive patient assessment, care planning, medication administration, patient/family counseling, and collaboration with physicians and the hospice team to ensure comfort and quality of life on a full-time basis while traveling to patient homes.
