Your Impact as a Hospice RN Case Manager
Develop and manage individualized Plans of Care with patients, families, physicians, and the IDG
Coordinate all hospice services and ensure seamless care across settings
Provide expert pain and symptom management
Educate patients and families on the Medicare Hospice Benefit and hands-on care
Supervise Home Health Aides and deliver high-quality care
Complete timely documentation and participate in admissions
What You’ll Bring
Current Oklahoma Registered Nurse license
At least 1 year of recent medical/surgical, oncology, home health, or hospice experience
Strong knowledge of pain and symptom management in terminally ill patients
Excellent communication, organization, and problem‑solving skills
Valid driver’s license, reliable transportation, and current auto insurance
