**Trustbridge Hospice,** a member of Empath Health, is seeking a motivated, relationship-driven **Physician Focused Hospice Representative** to join our team in **Lakeworth.**This **full-time,** “boots-on-the-ground” position works **Monday through Friday, 8:30 a.m. – 5:00 p.m., with weekend and after-hours availability as needed.** In this crucial community-facing role, the Hospice Representative strengthens Trustbridge’s presence by building meaningful relationships, educating healthcare partners, and driving access to compassionate hospice services.Since 1978, Trustbridge Hospice has cared for more than 200,000 South Florida families. As a community-based nonprofit, we provide 24/7 hospice and palliative care, along with caregiver support and bereavement services for families facing serious illness.**What You’ll Do*** Educate and promote the hospice concept and the benefits of Trustbridge programs to physicians, discharge planners, case managers, and other internal and external healthcare professionals.* Oversee accounts and open new ones, maintain strong relationships with hospital and facility staff, and increase referrals through consistent engagement.* Host occasional marketing events, facilitate meetings with clients, and build new relationships throughout the community.* Serve as a visible, boots-on-the-ground representative of Trustbridge in the Broward County service area.* Maintain accurate and timely documentation in the CRM; must be comfortable with technology and data entry.**Why Join Empath Health?*** **Fair, Competitive Pay:** Your work has value, and we reward it.
