Job description
Advocate Health is seeking a Registered Nurse in the 10422 IL Hospice Divisional - Patient Access to oversee hospice admissions and collaborate with the interdisciplinary team across Chicago area locations.
You will assess patients, obtain consent, verify prognosis, review insurance and coordinate care with IDT, while traveling between sites within the service area. Requires RN licensure and 2 years of direct patient care experience.
