Job description
St. Croix Hospice in New Albany, IN is seeking a Registered Nurse Case Manager to deliver 1:1 holistic home-based care. You’ll collaborate with an interdisciplinary team to develop care plans, guide end-of-life conversations, and ensure patient comfort with professionalism and empathy.
If you’re a compassionate RN who values meaningful patient connections and autonomy, we’d love to meet you. This is a full-time in-home care role with on-call duties and travel to patient homes.
