Job description
St. Croix Hospice is seeking an RN Case Manager to deliver 1:1, whole-person care to patients and families in their homes in Omaha, NE.
You will coordinate individualized care plans with an integrated care team, supporting families through complex decisions with empathy and professionalism. Join a compassionate family of caregivers dedicated to dignified end-of-life care, with a schedule from 8:00am to 5:00pm and on-call responsibilities.
