Job description
Generations Hospice in Wisconsin is seeking a compassionate Hospice RN Case Manager to provide patient-centered care with a smaller patient load and strong interdisciplinary support. The role includes work with an after-hours triage team and using Homecare Homebase EMR for documentation.
You will assess needs, create care plans aligned with patient wishes, administer medications, and educate families. Travel across Winnebago & Fond du Lac counties; schedule is Mon–Fri 8AM–5PM, with on-call
