Job description
UP Health System Home Care & Hospice, part of the LHC Group within the Optum family, seeks RN Case Managers to deliver compassionate hospice care, develop plans of care, and collaborate with the care team to promote comfort and dignity for patients and families.
We offer full-time and PRN opportunities, a $10,000 sign-on bonus, and support for Michigan RNs to transfer their license to Wisconsin.
