Job description
Trinity Health At Home is seeking a Hospice RN Case Manager in Michigan to deliver professional nursing care for hospice clients, manage assessments, care planning, and outcomes in collaboration with the physician and the IDT/IDG. The role requires active RN licensure in the state and at least one year of acute care experience.
The position offers a 15K sign-on bonus and a Friday–Sunday schedule from 8am to 9pm, with 36 hours worked and 40 paid per week, plus comprehensive benefits and ongoing
