Job description
Trinity Health At Home is seeking a Hospice RN Case Manager in Michigan to provide professional nursing care to hospice clients as prescribed by a physician and in line with state requirements.
You will develop and implement individualized plans of care, coordinate with the interdisciplinary team, and support patients and families through the hospice journey. RN licensure and 1 year acute care experience are required.
