Job description
Ascend Hospice is seeking a dedicated Hospice Registered Nurse Case Manager (RN) for Overland Park, KS. You will conduct initial assessments, re-evaluate nursing needs, and develop care plans in collaboration with the healthcare team to ensure continuous, well-coordinated care.
A 5k signing bonus is offered. Applicants must have an Associate's Degree in Nursing and a valid RN license, with 2 years of nursing experience; hospice experience is preferred.
