nWhat will you do as a Hospice RN Case Manager? Here are just a few of the responsibilities:
nnnUse clinical nursing knowledge, physical assessment, and procedural skills to deliver high quality patient care in the patient’s place of residence.
n
nnDevelop and oversee plan of care under the direction of the physician and ensure appropriate interdisciplinary involvement to ensure the best outcome for each patient.
n
nnPerform initial and comprehensive assessments on all patients in accordance with policies and procedures.
n
nnContinuous visits and interdisciplinary assessment of the patient.
n
nnDeliver quality care and completes clear, concise, and accurate clinical documentation.
n
nnProvide patient, caregiver and family counseling and education as indicated in the plan of care.
n
nnDevelop and continually update plan of care that identifies the care and services required to meet patient-specific needs.
n
n nWhat it takes to be an RN Case Manager with Care Initiatives Hospice:
nnnActive Iowa RN license.
n
nnActive CPR certification.
n
nnHospice or palliative care experience.
n
nnTwo (2) years of verifiable experience working as a nurse (RN/LPN), preferred.
n
nnCurrent driver's license, automobile liability insurance, and the ability to travel between multiple locations within your region.
n
nnHome access to the internet.
n
nnAbility to obtain Hospice and Palliative Registered Nurse Certification (CHPN) within 24 months of employment.
