Job description

Adoray Home Health and Hospice seeks a Hospice RN Case Manager to provide compassionate, autonomous care to patients and families in a team-based setting in Wisconsin. This full-time, exempt role includes a $2,000 sign-on bonus and eligibility for a benefits package.

The RN will coordinate with an interdisciplinary team to develop and implement patient care plans. Required are a Wisconsin RN license, BSN or two years hospice experience preferred, and strong communication skills.
