Job description
Ardencares in the United States is seeking a dedicated and compassionate Registered Nurse to join our hospice team as a Case Manager. You will coordinate patient care and oversee services throughout the end-of-life journey to ensure comfort, dignity, and quality of life.
You will collaborate with patients, families, and interdisciplinary health care providers to develop and implement personalized care plans, monitor progress, and adjust plans as needed to meet evolving needs.
