Job description
Assured Hospice of Olympia, part of LHC Group and UnitedHealth Group family, is seeking a Registered Nurse Case Manager to deliver hospice care and lead the Plan of Care for each patient. You will evaluate performance, collaborate with professionals, and implement treatment plans that promote well-being across physical, psychosocial, emotional, and spiritual domains.
