Job description
Regional Hospice And Home Care Of W is seeking a Registered Nurse Case Manager to coordinate patient care in the community and with Danbury Hospital as needed. You will partner with interdisciplinary team members to ensure compassionate, quality hospice and home health services aligned with our mission and patient/family needs.
The ideal candidate holds CT RN licensure, BSN preferred, and at least 2 years in acute care with hospice/home care experience; case management is strongly desired.
