Job description
Moments Hospice in Illinois is seeking an RN Case Manager to help make patients' final days, weeks, and months as comfortable as possible, while collaborating with the owners and embracing the company's entrepreneurial spirit.
You will conduct initial assessments, provide hands-on nursing care, educate patients and families, coordinate caseload care, and participate in interdisciplinary meetings to ensure high-quality planning.
