Job description
Heart n Home Hospice, part of the Optum family, seeks an RN Case Manager to deliver the Plan of Care for hospice patients and ensure quality care. You will evaluate performance and align with hospice policies to achieve excellent patient outcomes.
Responsibilities include initial eligibility assessment within 48 hours, comprehensive patient needs evaluation, care coordination, and daily collaboration with the care team, while upholding equal access and compassionate service.
