Job description
Gentiva is seeking a dedicated Hospice RN / RN Case Manager to deliver personalized hospice and palliative care in patients' homes across Crossville, TN. You will be the primary nurse guiding families through the care journey and coordinating with physicians, LPNs, CNAs, social workers, and chaplains.
You will assess conditions, implement and update plans of care, provide education to caregivers, and ensure accurate documentation while maintaining compassion and compliance.
