Job description
Ascend Hospice & Palliative Care PA is hiring an RN Case Manager to help with on-call/case management and patient visits. The role requires an active RN license and a commitment to compassionate end-of-life care.
This full-time position follows a Monday–Friday schedule (8:00 AM–5:30 PM) with travel to patient sites as needed, collaboration with a multidisciplinary team, and supervision of Hospice Aides.
