Job description
LiveWell Homecare Agency in Illinois seeks an experienced RN Case Manager to provide end-of-life nursing care, assess patient needs, develop care plans, and coordinate with a multidisciplinary team to ensure safe, patient-centered outcomes.
The role requires RN licensure, a Nursing Diploma/Associate’s degree, a valid driver’s license, and 2–5 years of Home Health experience; a Bachelor’s degree is preferred.
