Job description
Heart to Heart Hospice is seeking an RN Case Manager to deliver compassionate, patient-centered care for individuals with life-limiting illnesses in Indiana. You will assess needs, implement and adjust the Plan of Care, and coordinate with families and the care team to ensure high-quality support.
This full-time position offers benefits and requires a valid Indiana RN license, a driving license with reliable transportation, and at least two years of RN experience preferred in healthcare
