Job description
The Care Team Hospice is seeking a compassionate Hospice RN Case Manager to coordinate patient care, develop plans of care, and partner with physicians, caregivers, and the interdisciplinary team to ensure comfort and quality of life. You’ll complete visits, document in Homecare Homebase, and provide education to families, with a Monday–Friday schedule and on-call opportunities.
