About your hospice, oncology, palliative care, home health, or related nursing experience.

What you enjoy most about hospice case management.

Why you are passionate about caring for patients and families at the end of life.

What attracted you to Superior Hospice.

We personally review every application and look forward to learning more about you.

The Nurse Families Never Forget

Long after the medications have been administered and the paperwork is complete, families remember one thing most.

The nurse who showed up with compassion, confidence, and kindness during one of the most difficult seasons of their lives.

At Superior Hospice, our RN Case Managers do far more than coordinate care. They become trusted advocates, educators, and steady sources of comfort for patients and families navigating the end‑of‑life journey.

If you're looking for a career where meaningful relationships matter just as much as clinical excellence, we'd love to meet you.

Why You'll Thrive Here

Manage your own patient caseload while practicing at the top of your license.

Build lasting relationships with patients and families through consistent, compassionate care.

Collaborate with an experienced interdisciplinary team that truly supports one another.

Join a growing, locally led hospice backed by more than 30 years of home care leadership.

Supportive leadership that values your clinical judgment and encourages autonomy.

Tools, resources, and technology designed to help you spend more time caring for patients and less time navigating barriers.

Make a meaningful difference every day through personalized, patient‑centered hospice care.

What You'll Do

Serve as the primary Hospice RN Case Manager for your assigned patients and families.

Complete comprehensive nursing assessments and develop individualized plans of care.

Manage pain and symptoms while promoting comfort, dignity, and quality of life.

Educate patients, caregivers, and families throughout the hospice journey.

Coordinate care with physicians, nurse practitioners, social workers, chaplains, hospice aides, therapists, and community partners.

Respond to changing patient needs while providing compassionate guidance and clinical expertise.

Maintain accurate, timely EMR documentation while ensuring compliance with Medicare hospice regulations.

What You'll Bring

Active Minnesota Registered Nurse (RN) license.

Two or more years of hospice experience preferred, or a strong background in oncology, palliative care, home health, ICU, or other complex care settings.

Excellent assessment skills and independent clinical judgment.

Strong communication and relationship‑building abilities.

Compassionate, patient‑centered approach to end‑of‑life care.

Exceptional organization, time management, and critical thinking skills.

Reliable transportation and willingness to travel throughout your assigned territory.
