Job description
KeystoneCare is seeking a Hospice Registered Nurse (Philadelphia County) to join an interdisciplinary team delivering compassionate end-of-life care in home or hospice settings. This role emphasizes independent clinical decision-making within a supportive, patient-centered environment.
As a Hospice RN Case Manager, you will assess needs, plan care, manage symptoms, and coordinate with CNA/HHA staff to ensure dignity and comfort for patients and families.
