Job description
Gentiva Hospice is seeking a dedicated Hospice RN / RN Case Manager to lead nursing care for patients at home or in assisted living settings. You will perform comprehensive assessments, deliver skilled care, and coordinate with physicians, social workers, and chaplains to implement personalized plans of care.
You will educate families, support end-of-life goals, and document care accurately while maintaining compliance and quality standards.
