Job description
Altus Hospice is seeking a Full-time Hospice RN Case Manager (RNCM) to join our nursing team providing exceptional end-of-life care throughout the community. The role covers Victoria and surrounding counties with M-F schedule and rotating weekend on-call shifts.
You will deliver hands-on hospice nursing care, coordinate services, and guide patients and families through every step of their hospice journey, while collaborating with an interdisciplinary team to ensure The Best Care Possible.
