Job description
Concho Hearts Hospice is seeking a compassionate Hospice RN Case Manager for full-time work in Ennis and surrounding areas. You will deliver skilled nursing in patients' homes and facilities, manage a caseload, complete admissions and recertifications, and develop individualized plans of care.
