Job description
Care Initiatives Hospice in Iowa seeks an RN Case Manager to deliver comprehensive, compassionate care at patients' homes or care settings. You will perform assessments, develop plans of care, and coordinate interdisciplinary teams.
The role requires active Iowa RN license, CPR, hospice or palliative care experience, and ability to travel within the region. CHPN within 24 months is encouraged; competitive benefits and tuition reimbursement are offered.
