Job description
LilyCare Hospice of New Hampshire is seeking a full-time Hospice Registered Nurse Case Manager to join our clinical team in southern New Hampshire, based out of Bedford.
As the RN Case Manager, you will perform assessments, develop plans of care, manage symptoms, and coordinate with the interdisciplinary team. Requires NH (or compact) RN license, CPR, and at least 1 year of RN experience; valid driver's license; on-site role with a Monday–Friday schedule and rotating on-call.
