Job description
Heartland Hospice is seeking a dedicated RN Case Manager to lead in-home nursing care, assessments, and care plan development. You will collaborate with physicians, LPNs, CNAs, social workers, chaplains, and other team members to deliver compassionate, patient-centered hospice services.
You will educate patients and families, coordinate care across settings, maintain clinical documentation, and participate in on-call activities to ensure timely interventions and high-quality outcomes.
