Job description
St. Croix Hospice in Council Bluffs, IA, is seeking a Registered Nurse Case Manager to deliver individualized, holistic home-based care and lead end-of-life planning with empathy and clinical excellence.
You will develop personalized plans with patients and families, coordinate with the care team, and ensure safe medication administration, education, and smooth transitions. Schedule is 8:00am–5:00pm, full-time with on-call responsibilities.
