Job description
Heartland Hospice, part of Gentiva, is seeking a dedicated Hospice RN / RN Case Manager to lead compassionate home-based care for patients with life-limiting illnesses. You will assess needs, develop care plans, and guide families through the hospice journey with empathy.
You will collaborate with an interdisciplinary team, provide education, ensure documentation accuracy, and support transitions across care levels. A current IA RN license and hospice experience are preferred.
