Job description
Care Initiatives Hospice is seeking an RN Case Manager to deliver compassionate, patient- and family-centered care across homes, long-term care communities, assisted living, or hospitals. You will perform assessments, develop care plans with physicians, and coordinate interdisciplinary teams to meet physical, emotional, and spiritual needs.
Requirements include an Iowa RN license, CPR, hospice experience, and willingness to travel within the region.
