Job description
St. Croix Hospice is seeking a Registered Nurse Case Manager to deliver individualized, holistic home care.
You will collaborate with a care team to develop and update plans of care, lead meaningful end-of-life conversations, and ensure symptom management with empathy and clinical excellence. The role requires an RN with acute care experience and Hospice/Home Health background preferred, plus a valid driver’s license and reliable transportation to serve patients in their homes.
